Aberdeen City Community Health Partnership

      

 

Community Health Partnerships (CHPs) were established in April 2004 to build on the achievements of Local Health Care Co-operatives (LHCCs) and to make a direct and measurable improvement in local population health and to provide higher quality, accessible and joined up services to local communities.

CHPs are a key building block in the modernisation of NHS and joint service, with a vital role in partnership integration and service design. 

Partnership for Care states that CHPs have a new and more consistent and enhanced role in service planning and delivery working as part of decentralised but integrated health and social care systems.  Read about the Aberdeen City CHP (Community Health Partnership)

Aberdeen City CHP Structure

CHPs Will:

  • Ensure patients, carers and full range of health professionals are involved;
  • Establish a substantive partnership with local authority services (e.g. social work, housing, education and regeneration);
  • Have a greater responsibility and influence in the deployment of NHS Board resources;
  • Play a central role in service redesign locally;
  • Focus on integrating primary and specialist health services at local level;
  • Play a pivotal role in delivering health improvement
