| Community Health Partnerships (CHPs) were established in April 2004 to build on the achievements of Local Health Care Co-operatives (LHCCs) and to make a direct and measurable improvement in local population health and to provide higher quality, accessible and joined up services to local communities. CHPs are a key building block in the modernisation of NHS and joint service, with a vital role in partnership integration and service design. Partnership for Care states that CHPs have a new and more consistent and enhanced role in service planning and delivery working as part of decentralised but integrated health and social care systems.
