This service has been established to facilitate timely discharge home from hospital for patients who have had a stroke or an orthopaedic event (Trauma). The Multi-disciplinary Team will provide ongoing rehabilitation and support to patients in their own home for a defined period of time (4 to 6 weeks). The service is for people who live within Aberdeen City only. Patients requiring rehabilitation following the maximum 6 week intervention period will be referred on to the appropriate community services.

Over the next few weeks members of the team will be visiting the relevant wards within ARI, Woodend, Links Unit and partner agencies, to introduce themselves and provide information on the new service.The service will be operational from the 5 October 2009.
